MediSavers Program Details

1. Due to state and federal laws, points cannot be earned on some items, and points will not be earned in a transaction where rewards dollars are used.

2. Due to state and federal laws, points cannot be earned on some items. Points will not be awarded to anyone who currently is or was at any time in the 6 months prior to purchasing Pharmacy Items covered by Medicare, Medicaid, Tricare or any other government-funded healthcare program.

3. You must be a MediSaver Member to earn points and receive rewards and/or savings.

4. As a MediSaver member you may combine family members in your household under one MediSaver Membership.

5. MediSaver points will only be earned at a MediSav Homecare Pharmacy in Charleston, Arkansas or in Fort Smith, Arkansas. Points cannot be earned at any other location.

Terms and Conditions

MediSaver Rewards is a Savings and Rewards program offered by MediSav Homecare Pharmacies, Inc. to its customers. These terms and conditions form and agreement between each customer that participates in MediSavers Savings and Rewards Program and MediSav Homecare Pharmacies, Inc. By participating in the MediSaver Savings and Rewards Program, Members agrees to the terms and conditions, rules, regulations, policies, and procedures of the MediSaver Savings and Rewards Program available at medisav.com/medisavers. Each Member is responsible for remaining knowledgeable about the MediSaver Savings and Rewards Program terms and conditions. MediSav Homecare Pharmacies, Inc. reserves the right to change the terms and conditions of the MediSaver Savings and Rewards Program at any time without prior written notice. Communications about the MediSavers Savings and Rewards Program, including changes to the Program, will be posted on medisav.com/medisavers.

MediSavers Savings and Rewards Program is open to U.S. residents. If you are between the ages of 13 and 18 years old, you agree you have obtained permission from your parent or legal guardian to become a MediSaver in the MediSaver Rewards and Savings Program.

MediSaver Savings and Rewards Program memberships are available to individuals and to families (family members must reside at the same address). Member is responsible and liable for any tax consequences which may result from Membership’s participation in the Program. Membership is required at purchase to receive promotional pricing. Price adjustments are eligible only during the promotional pricing period.

Member should promptly notify the MediSaver Savings and Rewards Program of any changes in Personal Information such as name, address, telephone numbers, and email address either by visiting medisav.com/medisavers or by calling our office at 479-965-2160.

MediSav Homecare Pharmacies, Inc. reserves the right to cancel any MediSavers Savings and Rewards Memberships for which it has incomplete, inaccurate, false or fictitious Personal Information and all MediSavers Rewards points earned on that account will be forfeited as well as future savings opportunities that require membership in the MediSavers Savings and Rewards Program.

The MediSavers Savings and Rewards Program Membership enables members to access sales and promotional pricing, earn MediSavers Points on certain purchases and redeem points on futures purchases at MediSav Homecare Pharmacy locations previously listed in this document. From time to time, special program offers may be made that will not be available at all store locations.

Member must present his/her card or give the Membership name before the purchase transaction is completed in orders to earn points. Points have no fixed value, may not be redeemed for cash, and are not transferrable to any other membership number. Any fraudulent or unauthorized use of the Membership is strictly prohibited, and may result in termination or disqualification from the MediSavers Savings and Rewards Program. The card is property of MediSav Homecare Pharmacies, Inc. and it may be revokes at any time by MediSav at their sole discretion. Any unauthorized reproduction of the MediSaver Card may lead to legal prosecution and forfeiture of membership in the MediSavers Program and all Points earned.

Earning Points:

MediSavers Savings and Rewards Members may be offered promotions (“Point Promotions”) which will award Points with the purchase of specified products and/or services from a MediSav Homecare Pharmacy location during the promotion period. In addition Points may also be awarded for attending certain events or behaviors as determined by MediSav as Point Promotions. MediSav Homecare Pharmacies, Inc. reserves the right to apply point maximums at any time, at its sole discretion, without notice. Points will not be earned in a transaction where Rewards Dollars or Store Credit is used. Point Promotions will not apply to and Points cannot be earned on purchases of items or services that must be excluded by law as determined by MediSav Homecare Pharmacies, Inc. (Excluded Pharmacy Items) that are paid for in whole or in part by Medicare, Medicaid, TRICARE, or any other state, federal or government-funded program; Excluded Pharmacy Items purchased by a MediSaver Member who at anytime in the prior 6 months is or was a beneficiary of Medicare, Medicaid, TRICARE, or any other state, federal or government-funded healthcare program;

MediSaver Savings and Rewards Members will earn 100 points for every $1.00 spent all items that are not included in excluded pharmacy items. MediSav Homecare Pharmacies, Inc. reserves the right to add or delete items eligible for Points issuance at any time, at its sole discretion, without notice.

All Points awarded to the MediSaver Members account at the time of purchase. On a return and/or exchange the Points will be deducted from the Member’s account that was used for the original purchase.

Redeeming Points:

MediSav Homecare Pharmacies, Inc. will issue rewards coupons on a monthly basis. The coupon will be mailed directly to the Members address on file. Earned points will be converted into Rewards dollars each month. Any points over 5,000 will receive a reward coupon.

Rewards dollars will not be paid out in cash or Store Credit.

Redemption dollars may not be used to purchase gift cards. MediSav Homecare Pharmacies, Inc. may, at any time and without notice, change, eliminate, or terminate the Point earning and redemption procedures and offerings. MediSav, at its discretion, may not show full list of redemption tiers on marketing materials. Upon rewards dollars issuance, the points will immediately be deducted from the MediSaver’s account. Lost or used MediSavers Rewards Dollars cannot be replaced. Items returned that have been paid with Reward Dollars will be refunded to Member in the form of points credit on the members account.

By participating in the MediSaver Savings and Rewards Program, the Member agrees to allow MediSav to communicate via mail, email, phone and other channels. If permission is granted by the Member, MediSav may also communicate via mobile device. MediSav may use these channels to communicate account status, notification of Reward, Program changes, and offer special promotions, coupons and more. Members who are under the age of 18 or who choose to opt out of all marketing communications will receive only messages related to their account.

MediSav Homecare Pharmacies, Inc. will protect the privacy of Member information in accordance with our company privacy policies including the Notice of Privacy Practice. In the event that we elect to use Protected Health Information (PHI) for marketing purposes, we will ask you (or your parent or legal guardian) to approve such use by executing the HIPAA authorization form.
